چکیده
OBJECTIVE To illustrate the clinical features, laboratory findings, and management of anticonvulsant hypersensitivity syndrome (AHS), emphasizing the importance of recognizing its multiple clinical components and raising awareness of the cross-sensitivity among different anticonvulsants. CLINICAL PRESENTATION AND INTERVENTION Two cases of AHS due to carbamazepine and a combination of sodium valproate and lamotrigene are reported. Both patients presented within the first month of starting the new antiepileptic medication with fever, skin rashes, hematological abnormalities, and hepatitis. The offending antiepileptic drugs were immediately stopped in both cases. Skin rashes responded to intravenous immunoglobulin in case 1 and to intravenous hydrocortisone in case 2. CONCLUSION AHS is a serious, life-threatening condition. This report demonstrates that the most important steps in the management of AHS are to recognize the disorder, discontinue the offending antiepileptic drug, and provide supportive care in an inpatient setting and treat with benzodiazepines if seizures occur.

But anticonvulsant hypersensitivity syndrome (AHS) is a potentially fatal drug reaction with cutaneous and systemic reaction to the arene oxide-producing anticonvulsants, phenytoin, carbamazepine, and Phenobarbital sodium. The hall-mark features of this syndrome are: Fever, rash and lymphadenopathy. The epoxide hydrolase en...
متن کاملDrug neosensitization during anticonvulsant hypersensitivity syndrome.
Anticonvulsant hypersensitivity syndrome (AHS) is a rare, severe drug hypersensitivity reaction included in the drug-related rash with eosinophilia and systemic symptoms syndrome (DRESS), in which a transient state of immune suppression and reactivation of latent virus infections have been observed. We describe 5 patients who developed neosensitization to different drugs taken during a previous...

متن کاملAnticonvulsant Hypersensitivity Syndrome Associated with Epstein-Barr Virus Reactivation
We describe a 59-year-old female with severe anticonvulsant hypersensitivity syndrome (AHS) associated with Epstein- Barr virus (EBV) infection. The causative drug was speculated to be carbamazepine. Recurrent EBV infection was demonstrated by the presence of anti-EBV early antigen IgM antibodies and anti-EBV nuclear antigen IgG antibodies. To our knowledge, only one case of drug hypersensitivi...
متن کاملAnticonvulsant hypersensitivity syndrome secondary to lamotrigine mimicking a septic episode.
Dear Editor, Anticonvulsant hypersensitivity syndrome consists of the hallmark features of fever, rash, and lymphadenopathy and multiorgan involvement. We report a case of lamotrigine hypersensitivity that was diffi cult to diagnose because of comorbid systemic lupus eythrmatosus (SLE), and the presence of clinical features mimicking sepsis due to a zoonosis. The syndrome resolved with the disc...
